不同检测方法用于Rh(D)阴性确认检测比较

摘要:
[摘要] 目的 采用盐水法、凝聚胺法、抗人球蛋白法、微柱凝胶法4种检测方法用于Rh(D)阴性确认检测的比较及应用效果分析.方法 选取2012年1月至2013年12月期间云南省第一人民医院输血科Rh(D)血型鉴定中初筛结果为阴性的标本96例,同时选用4种方法学进行Rh(D)阴性确认和分型实验.结果 盐水法检测发现96例为Rh(D)确认阴性结果;凝聚胺法检测发现84例为Rh(D)确认阴性结果;抗人球蛋白法检测发现82例为Rh(D)确认阴性结果;微柱凝胶法检测发现81例为Rh(D)确认阴性结果.结论 盐水法检测操作简单,但特异性差.凝聚胺法快速、简便,但会漏检抗体效价较弱的IgG抗体;抗人球蛋白法操作复杂,但灵敏度高、特异性好;微柱凝胶法对检测血清中的IgG抗体具有灵敏度高、特异性强、操作简便的特点,但成本较高.在实际工作中,多种方法的联合应用更能保证结果的准确性.

Comparison of Different Methods Used in Rh(D) Negative Confirmation Test

[Abstract]Objective To compare and discuss the different results among four kinds of experimental methods(saline water method,Antiglobulin method,Polybrene method ,Microcolumn gel test) in Rh(D) negative confirmation experiment, and analyze their effects . Methods We selected 96 cases whose results of Rh(D) blood typing in the early screening specimen were negative in the department of blood transfusion of the first people's hospital of yunnan province between January 2012 and December 2012. And we used saline wator method,antiglobulin method,polybrene method, microcolumn gel test to make Rh(D) the negative confirmation experiment and typing experiments at the same time. Results 96 cases were all confirmed the negative results through Saline method. 84 cases of 96 were confirmed the negative results through Polybrene method. 82 cases of 96 were confirmed the negative results through Anti-human globulin test. 81 cases of 96 were confirmed the negative results through Microcolumn gel test. Conclusions Saline water method is simple but has poor specificity.Polybrene method is simple and rapid, but it will miss the IgG antibody of weaker antibody titer. The operation of antiglobulin method is complex, but its sensitivity and specificity are good. Microcolumn gel test for detection of IgG antibody in the serum has the characteristics of high sensitivity,strong specificity,and simple operation . In practice, combination of various methods can more guarantee the accuracy of the results.
